Independent studio Ice-Pick Lodge has officially announced that Pathologic 3 will arrive on PC via Steam on January 9, 2026, distributed by HypeTrain Digital. The title marks a return to one of the most enigmatic narrative universes in contemporary gaming, resuming the role of physician and scientist Daniil Dankovsky, who must face a devastating epidemic in the mysterious Town-on-Gorkhon.
The playable demo, released as part of the October Steam Next Fest, allows players to experience for the first time the setting, psychological mechanics, and ethical dilemmas that will define this new installment. With an oppressive atmosphere and a narrative approach focused on collective survival, Pathologic 3 aims to establish itself as one of the most ambitious proposals of next year.
A story of epidemic, ethical dilemmas, and irreversible decisions
The plot places Dankovsky in an Eastern city ravaged by an unknown disease. In just 12 days, the protagonist must try to save the population or witness its collapse. The game presents a constant struggle against time, where every choice affects the life or death of the inhabitants and the fate of the city.
The player must ration medicine, decide whom to save, and determine how to respond to riots or social crises. These decisions not only alter the course of the story, but also reveal the main character's moral boundaries. In this sense, Pathologic 3 distances itself from traditional action narratives and moves closer to a philosophical and emotional experience, where the human cost of each decision carries narrative weight.
